Apply for or Renew your DSWV Card

Disaster Service Worker Volunteer cards are valid for two years.

To renew or apply for a new DSWV card, you must complete the following*: to renew.*

Required Online Training from FEMA:

  1. IS-100 – Introduction to Incident Command System, ICS-100
  2. (Optional) IS-317.A – Introduction to Community Emergency Response Team (CERTs)
  3. IS-700 – National Incident Management System (NIMS) An Introduction

Required Forms:

  1. San Diego County Code of Conduct
    • Fill out the form
    • Scan and email to: CMartinez@oceansideca.org (Or you can hand it to a member of the CERT Board)
  2. Oceanside CERT Application and Hold Harmless Agreement Renewal
    • Fill out the form (PDF)
    • Scan and email to: CMartinez@oceansideca.org (Or you can hand it to a member of the CERT Board)
  3. Oceanside CERT SOP Acknowledgment Form
    • Fill out the form (PDF)
    • Scan and email to: CMartinez@oceansideca.org (Or you can hand it to a member of the CERT Board)
  4. San Diego County DSWV Form

Required Attendance:

  1. Attend at least two approved Oceanside CERT Training events, per year, during their DSWV card cycle
    • Two approved training events consists of our quarterly drills or participation in a county drill. If your card expired and you haven’t been to two training events in the last 12 months you are not eligible for renewal.

Required Live-Scan**:

  1. Successfully complete a Live-Scan background check with the City of Oceanside Human Resources Department
    • An appointment is necessary with Oceanside Human Resources to take the live scan
      • To setup an appointment, call 760-435-4305.
      • Leave your name, number, and a couple of days and times that will work for you.
      • You will receive a call back with your appointment time.
    • It takes approximately 15 minutes and only has to be taken once
    • If you have already had a Live-Scan by City of Oceanside HR Department, you don’t have to do it again
      • Note, this is not the same as the old “My Background Check.com” we used to do

Once all the paperwork has been submitted to the County it takes approximately two weeks to process.

Recently graduated CERT members who have completed the above requirements will also be considered DSWV CERT member.
